PSA Oriental Mindoro Replaces PhilID with Damaged 
and/or Defective Front Facing Photograph

 

 

The Philippine Statistics Authority (PSA) Oriental Mindoro Provincial Office has formally begun accepting requests for replacement of PhilIDs with damaged and/or defective front facing photographs. This is to guarantee that PhilID owners can avail the PhilSys-enabled services. 

To file a request for the replacement of PhilID, the cardholder must accomplish the request form for Replacement of PhilID and/or letter bearing the PSN and surrender the PhilID Card with damaged and/or defective photo. An acknowledgement receipt form is given to the requester which will be required in claiming the replacement PhilID. The cardholder is advised to wait for the text message from PSA for the release of the new PhilID.

Presently, cardholders who seek to replace their PhilIDs with damaged photos may visit the PSA-Based Registration Center located at Nuciti Central Calapan, Brgy.Camilmil, Calapan City. This remains accessible to the public for rendering all types of PhilSys transactions. Mobile registration activities are also conducted in different cities/municipalities within the Province of Oriental Mindoro. Among these, are the municipalities of Puerto Galera, Baco, Naujan, Victoria, Socorro, Pola, Pinamalayan, Gloria, Bansud, Bongabong, Roxas, Mansalay and Bulalacao.
